Fill a Form

Before you start

  • The form stage must be active on your Application Timeline. Stages above it must be completed first.
  • Some forms are simple questionnaires; others are quizzes with scored questions — you'll see your score after submission.
  • If a stage is missing a form altogether, the page will show No Form Configured — contact the admissions office.

Open the form

  1. From your Application Timeline, find the active form stage and click Open Form.
  2. The form opens on a new page with the campaign name in a smaller line above the stage title.
  3. Use the back arrow at the top-left of the form page to return to the timeline at any time — your answers stay saved.

Answer and save as you go

The form is rendered question by question (or in a single long page, depending on how the institution built it). As you fill in answers:

  • The form auto-saves every time you finish a field — there is no manual save button.
  • You can close the tab, sign out, and come back later. When you reopen the stage your answers will still be there.
  • Required questions are marked. The form prevents you from submitting until they're filled.
  • If a question has a file upload, drag the file into the upload area or click to browse — accepted types are listed under the input.

Submit the form

When you reach the last page, a Submit (or Complete) button appears. Click it to send your answers in.

After submitting:

  • You return to your Application Timeline.
  • The stage moves to one of these states, depending on how the institution configured it:
    • Completed (green check) — the stage is done and the next one unlocks straight away.
    • Submitted / Under review (hourglass) — the admissions team is reviewing the form before it counts as complete. No action from you until they decide.
    • Graded (for quizzes) — see Quiz forms below.

What happens after you submit

If your stage shows Under review, the admissions team has three options:

You'll get a notification when any of these happens.

Respond to a Revision Required notice

If the admissions team sends the form back for changes, the stage shows Revision Required and a Revise Form button.

Click Revise Form to open the revision view. It looks different from the original form:

  1. A Submission Progress bar at the top shows how many questions are Verified, Pending, or Need revision.
  2. Each question is its own card with a status:
    • Verified (green) — accepted, no changes needed.
    • Pending (amber) — not yet reviewed.
    • Revision (red) — the team has flagged this question. Read the comment they left.
  3. Click Edit on any card you need to change. A dialog opens with the original answer; update it and click Save.
  4. When everything is fixed, click Submit Revisions at the bottom to resend.

You can only submit once you've actually changed at least one of the flagged questions.

View what you submitted

After a form is submitted you can re-open it in read-only mode to check what you sent:

  • From the timeline, click the View link on the stage's Form submitted successfully badge.
  • A blue "Viewing Submission" banner appears at the top so you know you can't change anything.
  • Use the back arrow to return to the timeline.

Quiz forms

If the form is a quiz (i.e. it has scored questions), the page after submission is a Quiz Results screen with:

  • Your total score out of the maximum possible.
  • A percentage and a pass / fail badge.
  • A list of every question with the score you got on each.

Quiz results are visible to you and to the admissions team. The stage status reflects the result — Completed if you passed, Rejected if you failed (when the form is configured to gate on the score).
